 Parents - summer is approaching! That means sports and camp physicals season is upon us!  Many parents opt to get their children’s camp or sports physicals performed early in the summer to avoid the rush (and long lines) of last-minute physicals right before summer camp and sports season. Many parents know they need to get sports physicals for children to be eligible to participate in camp and school athletics, but there’s always a few lingering questions.

Below we’ve answered the top 10 questions that we typically get during sport and camp physical season.

Can ChoiceOne verify my child’s immunizations during a sports or camp physical evaluation?

Unfortunately, we are unable to verify your child’s immunizations unless they were performed at a ChoiceOne center. Your child can still get their sports or camp physical performed at ChoiceOne. We recommend contacting your doctor’s office for verification of your child’s immunizations and ask that you bring that with you. 


If my child is missing vaccinations, can they get them during a sports or camp physicals evaluation?

At ChoiceOne, the vaccinations we offer are not all of the vaccinations required as part of a standard childhood immunization list.  We highly recommend utilizing your child’s primary care doctor to stay up-to-date on your child’s immunizations. 


What should I bring to a sports or camp physical evaluation?

Be sure to bring the physical form(s) that are required by your child’s school. If your child wears glasses or contacts, please bring those with you as well. Is a camp or sports physical the same as an annual physical?

No.  Sports or camp physicals are not as comprehensive as an annual physical and should not replace an annual check-up with your child’s primary care physician.


Can my child get a sports or camp physical without insurance?

Yes! The price advertised is the same regardless if you do or do not have insurance.  If you have insurance, you can always submit your receipt to your insurance provider for reimbursement.


Which ChoiceOne center offers sports and camp physicals evaluations?

They all do! You can simply walk-in, or save your spot in line by checking in online. Our centers are open every day from 8 a.m. to 8 p.m.


My child needs an EKG.  Does a sports or camp physical evaluation include that?

A standard sports or camp physical does not include an EKG; however, ChoiceOne centers are equipped with EKG machines.If you need to add an EKG to your sports or camp physical, you may do so for an additional fee.


Can I ask additional questions during the sports physical regarding my child’s health that do not relate to sports participation?

We recommend you schedule an appointment with your primary care provider to ask additional questions. These screenings are not intended to replace an annual comprehensive physical exam.


Once my child's sports or camp physical form is completed, what's next?

Completed sports or camp physical forms should be given to the school nurse, athletic coach, camp instructor, or any other individual, as directed by the school or camp. 


How long will the exam take?

The sports or camp physical exams usually take about 30 minutes. Our staff members will do their best to
